In dental applications, the contact between the metal implant and the receiving living tissue is made through the oxide layer on the implant surface, which allows the osseointegration process. In dentistry, the passive film formed on titanium seems to be more stable and protective than that formed on the Ti alloys, customarily used in other medical applications. Corrosion of titanium alloys in the mouth can result from the presence of a number of corrosive species, such as the hydrogen ion (H(+)), sulfide compounds (S(2-)), dissolved oxygen (O(2)) and Cl(-) and can result in the release of Ti(4+) ions that, in turn, brings about the reduction of alkaline phosphatase activity of osteoblastic cells. The present study reports a time-dependent electrochemical corrosion study of titanium in contact with the following biologically relevant solutions: (i) SBF (simulating the inorganic part of human plasma), (ii) SBF with added ovalbumin (a protein simulating the post-implant environment) and (iii) human plasma. To the best of the authors' knowledge, this is the first report on the corrosion of Ti in human plasma. The electrochemical measurements are based on electrochemical impedance spectrometry. Impedance spectra were interpreted on the basis of the equivalent-circuit approach and estimates of the time-variation of oxide film thickness and resistance were computed. Surface Raman spectroscopy was used to characterise the structure of as-anodised and corroded TiO(2) films: the effects of phosphate and organic incorporation were highlighted. EIS and surface Raman measurements have demonstrated that the corrosion resistance of the oxide films formed on Ti is strongly affected by the presence of biomolecules in the chloride- and phosphate-based aqueous solution. In particular, ovalbumin increases corrosion performance and human plasma is found to be remarkably more aggressive in comparison to SBF. These results suggest some caution in extrapolating corrosion results obtained in simulated biological fluids to the actual behaviour in vivo.

Titanium and its alloys are used in dentistry for implants because of its unique combination of chemical, physical, and biological properties. They are used in dentistry in cast and wrought form. The long term presence of corrosion reaction products and ongoing corrosion lead to fractures of the alloy-abutment interface, abutment, or implant body. The combination of stress, corrosion, and bacteria contribute to implant failure. This article highlights a review of the various aspects of corrosion and biocompatibility of dental titanium implants as well as suprastructures. This knowledge will also be helpful in exploring possible research strategies for probing the biological properties of materials.

Abstract Vari metalli con potenziali effetti tossici sono contenuti in materiali molto usati in campo odontoiatrico, per cure conservative, protesi e chirurgia implantare e maxillo-facciale. Ricordiamo cromo, cobalto, titanio, nichel ed alluminio presenti in alcune leghe per protesi, e nella gran maggioranza degli impianti dentari endossei, nonché il mercurio contenuto nell'amalgama per restauri dentari. Il crescente impiego clinico di impianti dentari, ha rivolto l'attenzione di numerosi ricercatori sul possibile rilascio nell'organismo ospite di componenti metalliche degli impianti, e della loro eventuale diffusione a distanza con accumulo in organi bersaglio. Tuttavia, le ricerche sin qui eseguite con varie tecniche di analisi, sia in pazienti che in animali, hanno condotto a risultati non concordi. Avendo la disponibilità di sonde a protoni ed a raggi X ed una microsonda a protoni e raggi X dei Laboratori Nazionali di Legnaro, dell'Istituto Nazionale di Fisica Nucleare, ci si propone di eseguire studi che contribuiscano a chiarire le tematiche sopra illustrate, effettuando la ricerca, il dosaggio e la mappatura soprattutto del titanio, del cromo, cobalto e mercurio, metalli di vasto uso odontoiatrico. Si faranno dosaggi del contenuto di questi elementi nei tessuti orali di pazienti portatori di protesi, impianti ed amalgama, in trattamento presso la Clinica Odontoiatrica di Padova, ed anche nella saliva, nelle urine e nel sangue. Coordinatore Scientifico del Programma di Ricerca Pietro PASSI Università degli Studi di PADOVA Obiettivo del Programma di Ricerca Riguardo gli impianti endossei, l'obiettivo è di verificare se, in che misura ed quali condizioni possa avvenire rilascio di componenti metalliche. L'indagine microPIXE effettuata su sezioni ottenute con tecnica di taglio e molaggio, comprendenti impianto e tessuti adiacenti, consente uno studio ottimale dell'interfaccia impianto-tessuto, disegnando la mappa degli elementi presenti nel campo esaminato. Questa analisi consente di vedere le modalità di diffusione degli elementi, in base alla loro disposizione superficiale, e costituisce un prezioso ausilio nello studio del comportamento chimico-fisico della superficie degli impianti endossei. Infatti, la morfologia di superficie, sia a livello microscopico che macroscopico, è una delle caratteristiche principali che differenziano i numerosi tipi di impianti utilizzati oggi. Abbiamo infatti manufatti "di tornio", mentre altri sono sottoposti a trattamenti che ne irruvidiscono la superficie, mediante sabbiatura, mordenzatura acida, rivestimento al plasma-spray e altri: la tendenza attuale va nella direzione di produrre impianti a superficie mossa, che sembrano produrre un legame con l'osso più veloce e tenace. Tuttavia, la presenza di micronicchie superficiali, conseguenti alla ruvidità, può produrre fenomeni di corrosione differenziale a causa della diversa concentrazione di ossigeno, ed intensificare la diffusione anche dei metalli passivanti, come il titanio. Risultati parziali attesi Dalla prima fase ci sia attende soprattutto una messa a punto assai precisa della preparazione dei bersagli per le misure mediante le tecniche PIXE e XRF, che sarà resa possibile dal confronto con i risultati ottenuti mediante spettroscopia. Da ricordare che la spettroscopia, specie con la tecnica FI-(VG)-ICP-MS ovvero Flow Injection-(Vapour Generation)-Inductively Coupled Plasma- Mass Spectrometer, che si intende utilizzare presso laboratori esterni, è molto collaudata ed affidabile; tuttavia è costosa, e consente di dosare un singolo elemento alla volta, mentre con PIXE ed XRF si può avere la rilevazione ed il dosaggio simultaneo di tutti gli elementi oggetto dell'indagine, oltre che la loro disposizione superficiale (PIXE microbeam). Pertanto, queste ultime tecniche si prospettano assai idonee ad indagini anche su vasta scala e per un gran numero di elementi. Ci si attende inoltre una prima valutazione sull'utilità del confronto tra quadri istologici e mappe degli elementi ottenuti con PIXE microbeam. Si otterranno anche utili indicazioni se la saliva sia un mezzo idoneo per rilevare metalli rilasciati da restauri o protesi endorali, argomento che è tuttora oggetto di discussione.Si ritiene di poter raccogliere una quantità sufficiente di dati per stabilire: -quali siano le migliori condizioni di impiego delle apparecchiature a fasci di particelle e raggi X utilizzati, ed i relativi limiti di rilevazione per gli elementi studiati. >>>
Durata 24 mesi Base di partenza scientifica nazionale o internazionale Sono stati descritti ed ipotizzati vari effetti avversi derivanti da metalli ampiamente utilizzati in campo odontoiatrico, in particolare dal mercurio contenuto negli amalgami, il nichel, il cromo, l'alluminio ed il vanadio presenti in numerose leghe per uso protesico ed implantologico. Per quanto concerne l'amalgama d'argento e i possibili effetti negativi del mercurio, le ricerche sin qui eseguite hanno portato a risultati discordanti. In ratti sottoposti ad inalazione di limatura di amalgama per un'ora (proveniente dal fresaggio di un blocco di materiale) si sono riscontrate alterazioni renali, ed accumulo del mercurio nel rene e nei testicoli (Musajo e Coll., 1988). Seidler e Coll. (1996) hanno studiato oltre 1.000 pazienti affetti da morbo di Parkinson in Germania, analizzando le loro abitudini alimentari e l' eventuale esposizione a sostanze tossiche. A detta di questi Autori, vi sarebbe una correlazione statisticamente significativa tra ricostruzioni dentarie in amalgama e malattia di Parkinson, nel gruppo di pazienti in esame. Il contenuto di Hg nelle urine sembra essere un indicatore più attendibile rispetto al tasso ematico, in quanto il mercurio tende a concentrarsi nei reni. Anche la valutazione quantitativa di questo elemento nella saliva è da ritenersi di estrema utilità. Alcune ricerche, infatti, hanno evidenziato una correlazione tra numero ed estensione dei reaturi dentali in amalgama e livelli di Hg salivare (Lygre et al., 1999) >>>
